This report is annually published for the state of California by the California Department of Public Health (CDPH) in collaboration with the California of Local Health Officers. It presents public health data that can be directly compared to national standards and populations of similar compositions. Data includes health status indicators, mortality trends, and mortality indicators.
Providence Mission Hospital developed a three year Community Health Improvement Plan (CHIP) to respond to the three priorities identified by the community health needs assessment. This report summarizes the CHIP planning process with community benefit initiatives and Evaluation plan.
Through a collaborative process engaging community health committee members, the Director of Community Health Investment, primary and secondary data, the three significant priorities identified by the 2021 community health needs assessment and the areas that St Joseph Hospital will focus on for the 2021-2023 community benefits efforts are: mental health; health care access; and homelessness and housing.
